Adult Participant Waiver

Participant Suitability

Please choose the experience with your guests in mind. Some formats may include suspense, surprise, darkness, theatrical tension, simulated crime themes, or sudden sound and visual effects. Signature experiences are designed for participants aged 10 and over. The client is responsible for assessing participant suitability — including children — and for informing ARTHUS TAILOR in advance of relevant sensitivities or concerns. Content can be adapted when these needs are disclosed before production.

To the fullest extent permitted by applicable law, ARTHUS TAILOR is not responsible for ordinary emotional reactions reasonably associated with the disclosed nature of the selected experience, including fear, distress or temporary sleep disturbance, except where liability cannot legally be excluded. Final bookings are subject to the approved proposal and applicable terms and conditions.

Voluntary Participation and Assumption of Risk

I understand that ARTHUS TAILOR experiences may involve interactive games, movement, props, temporary installations, physical or strategic challenges, darkness or reduced lighting, theatrical sound, visual effects, simulated crime or suspense themes, and other supervised entertainment activities.

I voluntarily choose to participate. I understand that participation may involve risks that cannot be completely eliminated, including slips, trips, falls, collisions with objects or other participants, minor cuts or bruises, startle reactions, emotional discomfort, and other risks inherent in interactive activities.

Safety and Participant Responsibilities

• I will follow reasonable instructions given by ARTHUS TAILOR personnel.
• I will not deliberately damage equipment, props, furnishings or the event venue.
• I will not participate while impaired by alcohol, drugs or any condition that makes participation unsafe.
• I will stop participating and notify ARTHUS TAILOR if I feel unsafe, unwell or unable to continue.
• I will disclose in advance any relevant limitation or sensitivity that I reasonably believe may affect safe participation.

Release of Liability

To the fullest extent permitted by Florida law, I release and hold harmless ARTHUS TAILOR LLC and its owners, members, employees, contractors and agents from claims for personal injury, death or property damage arising from the ordinary and inherent risks of participation and, to the extent legally enforceable, from claims arising from the ordinary negligence of ARTHUS TAILOR LLC or the released parties in connection with the activity.

This release does not apply to gross negligence, reckless or intentional misconduct, or any liability that cannot lawfully be waived.

Property Damage

I understand that I may be financially responsible for damage I intentionally cause, or damage resulting from my failure to follow reasonable instructions, to ARTHUS TAILOR equipment, props, or the event venue, to the extent permitted by law.

Emergency Assistance

If reasonably necessary during an emergency, I authorize ARTHUS TAILOR personnel to contact emergency services. I understand that ARTHUS TAILOR does not provide medical care and that any medical costs remain my responsibility except where otherwise required by law.

Acknowledgment

I confirm that I have read this waiver, understand it, have had the opportunity to ask questions, and voluntarily agree to its terms.

Minor Participant Agreement

Notice to the Minor Child’s Natural Guardian

For a parental waiver relying on Florida Statute §744.301(3), Florida law requires the following statutory notice. The wording below is legal language and does not describe the expected intensity of the experience.

Parent / Natural Guardian Consent

I am the parent or natural guardian authorized to sign on behalf of the minor participant identified in this form. I give permission for the minor to participate in the selected ARTHUS TAILOR experience.

Participant Suitability

I understand that some ARTHUS TAILOR experiences may include suspense, surprise, darkness, theatrical tension, simulated crime themes, sudden sound or visual effects, movement, props, temporary installations and supervised interactive challenges.

I have considered the child’s age, maturity, sensitivities and ability to participate safely. I have informed ARTHUS TAILOR in advance of relevant concerns that may reasonably require adaptation. I understand that content can only be adapted when those needs are disclosed before production.

Assumption Of Inherent Risks

On behalf of the minor, and to the extent permitted by Florida Statute §744.301(3), I acknowledge and accept the inherent risks of the activity, including risks that are characteristic of or integral to interactive entertainment and that cannot be eliminated even when reasonable care is used.

Safety

• The minor must follow reasonable instructions given by ARTHUS TAILOR personnel.
• The minor must not intentionally misuse props, equipment or installations.
• The minor should stop participating and notify an adult if the activity feels unsafe or if the minor feels unwell.
• The parent or natural guardian must disclose relevant sensitivities or limitations before the event when reasonably necessary for safe participation.

Release Limited To What Florida Law Allows

This waiver is intended to release only those claims that a parent or natural guardian may lawfully waive in advance for a minor under Florida law, including claims arising from inherent risks as provided by Florida Statute §744.301(3). It does not waive rights or claims that Florida law does not permit a parent or guardian to waive.

I confirm that I have read this form, understand the nature of the activity, have had the opportunity to ask questions, and voluntarily authorize the minor’s participation subject to the terms above.
